South Country Health Alliance (SCHA) is a county-based health plan provider that works with individuals in 12 counties in Minnesota.  These counties include Todd, Wadena, Morrison, Kanabec, Sibley, Brown, Goodhue, Wabasha, Waseca, Steele, Dodge and Freeborn.  Founded in 2001, South Country Health Alliance aims to help their members be as healthy as possible while working with local providers and agencies to provide quality health services to their clients.  SCHA provides both Medicaid and Medicare programs.  Their Medicaid programs include Medical Assistance (PMAP), MinnesotaCare, Minnesota Senior Care Plus (MSC+), SingleCare (SNBC) and SharedCare (SNBC).  The Medicare programs include SeniorCare Complete (HMO SNP) and AbilityCare (HMO SNP).  Members are required to meet qualifying standards to be eligible for any program through South Country Health Alliance.  

B507 created a QlikView data dashboard for South Country Health Alliance, SCHA that has filtering and sorting capabilities. The users of the dashboard will be able to easily identify and filter client risk scores based on gender, age, disability and chronic condition. South Country Health Alliance is able to quickly identify client risk scores and clients without sufficient encounter documentation.  More accurately, those members who may be missing a diagnosis code/s. Risk score information is used to maximize capitation rates for South Country Health Alliance.
